when a human is forming in the womb they 100% of the time will begin as female. they will remain developing as female for a while before some of them will alter and develop fully as male. (this is why even males have nipples)
sometimes there can be problems in development resulting in one of the fetuses not fully transforming when switching to male, this can result in body developing mixed or (more commonly) developing with the apperance of being fully on of them but mentally being mixed.

gender dysphoria probably occurs between ages 0-7 like other identity disorders do, there are actual physiological differences between female and male brains such as in the Kappa opioid receptor but we find that it really isn't too big of a deal. I don't think anyone has opened up a trans person's brain and looked at the receptor expression to see if it actually is a physiological difference.
